Create forms in CHUMS*

  1. Forms Screen To create forms click on the forms icon

  2. Screen Shot 2023-05-09 at 3 20 40 PM Click the “+” button to add a form

  4. Screen Shot 2023-05-09 at 3 23 35 PM Associate the form with “People” or “Stand Alone”. The “people” form will show up in the edit box of each individual. The “stand alone” form will produce a link which can be used in your web page

  5. Screen Shot 2023-05-09 at 3 24 51 PM You will now see your form. Click on the form name (to rename click on the edit button)

  6. Screen Shot 2023-05-09 at 3 51 43 PM Click the “+” to add a question. Choose from a variety of question types. Click save. The form will appear to the left.

  7. Screen Shot 2023-05-09 at 3 28 42 PM You are now ready to use the form. Click on the persons name and you will see the title of the form below their info. Click on the name of the form and fill in the blanks.

Create a Stand Alone Form

  1. Stand alone 1 Name your form, Choose “Stand Alone” in the “Associate with” box, public or private access, and time frame or not

  2. Stand alone 2 Add questions to your form

  3. Stand alone 3 Choose from the different types of questions available under “Provider”

  4. Stand alone 4 Now navigate to your B1.Church website builder page and create a page for your form or insert it into a page you have already created

  5. Stand alone 5 In the “Church Specific” box choose the “Form” element

  6. Stand alone 6 In the “Edit Element” box choose one of your forms (In this case we have only made one form so that’s all that shows up here)

  7. Stand alone 7 You can see the results of your form responses under “Form Submissions”

  8. Form e-mail If you want to have the completed forms e-mailed to you go to “Form Members” and turn on “Email Notification” You can also add other people with authority to change the form or view form results here
